Our network of providers are all about you and the local communities we live in. If you are a new patient deciding on whether or not one of our providers can help, please call or chat with us now and an administrator and/or doctor will get back to you with your questions and concerns.

    3. Convenience & Coverage: We understand the NOVA traffic and your busy lifestyles.  A provider will always be available as we currently have 5 locations to choose from. Hours vary by each provider and most major insurances accepted.

      1. Provider Type (Chiropractic, Physical Therapy, Acupuncture, Massage) and/or provider name you wish to see.
      2. Location Preference (Ashburn Route 7, Ashburn Waxpool, Ashburn/Broadlands, Leesburg, Sterling or Reston, VA)
      3. Referral source (please let us know whom to thank)
      4. Insurance information (Member ID and Group #)
      5. Day and time of desired first appointment.

Fill out forms at home

Click here to access our paperwork which can be downloaded and completed at home. Please obtain your necessary referrals beforehand.  Bring all paperwork, your drivers license, and insurance ID with you to your appointment.   If you have previous x-ray/MRI images, medical reports, or doctors notes, please bring those as well.

*Please come 15-20 mins early to your scheduled appointment if you do not fill out your paperwork in advance.
