Call to schedule for an appointment
Please let our front desk know know the following:
- preferred location (Ashburn or Reston)
- provider type (Chiropractic, Acupuncture, Massage – Acupuncture and massage are only available at our Ashburn office at this time.
- referral source (How did you hear about our practice? Whom may we thank for your referral?)
- insurance information
- day and time of desired first appointment!

Fill out forms at home
Click here to access our paperwork that can be downloaded and completed at home. Please bring all of your paperwork with you and arrive on-time to ensure adequate time for processing your paperwork. If you do not have your paperwork filled-out ahead of time, please arrive 15 minutes prior to your scheduled time so you may complete the paperwork in our office.
Miscellaneous
Please make sure you also bring your photo ID, insurance card(s), and referral, if applicable. If you have previous x-ray/MRI images, medical reports, or doctors notes, please bring those as well.
